This form consists of 2 pages that can be printed on your computer's printer and used to place an order by fax or by mail. You may use your own order form if desired. To print,use the "print" command in the "file" menu of your browser. |

| Note: If you are having margin problems printing from your browser, evidenced by parts of images being cut off and appearing on an unwanted extra page, try adjusting the page margin settings in the "page setup" menu in the "file" menu of your browser. By reducing the default settings to "0.20" (or even less) you will find that most internet printouts from your browser will be well behaved.
